This form is intended for prospective applicants who already have a Diploma or A.S. Degree in Diagnostic Medical Sonography or Ultrasound Technology and who are exploring UMI's Bachelor of Science in DMS program.
If you do NOT have a DMS Diploma or A.S. Degree, please schedule your Diploma / A.S. Degree Info Session instead.
